RN FIELD CASE MANAGER | DETAILS AND COMPENSATION:

• Location: Virginia Beach, VA 23452 – 100% on-site, filed work/home health setting
• • Covering: Chesapeake, Virginia Beach, Portsmouth, Suffolk, and Norfolk
• Payrate: $56.15/hr + $0.54/mile
• Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM

RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS:

The RN Field Case Manager coordinates comprehensive care for high-risk members by performing clinical assessments and developing individualized treatment plans in collaboration with healthcare providers. The role focuses on managing transitions of care and addressing social determinants of health—such as housing and transportation—to improve patient outcomes and ensure cost-efficient service delivery.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN FIELD CASE MANAGER | RESPONSIBILITIES:

• Comprehensive Assessment: Perform detailed assessments of high-risk members and identify opportunities for health promotion and illness prevention.
• Care Coordination: Collaborate with PCPs, social workers, and discharge planners to implement individualized care plans.
• Resource Management: Identify cost-efficient levels of care and arrange non-medical supports, such as housing and transportation, to ensure treatment compliance.
• Patient Advocacy: Protect patient privacy and confidentiality while intervening quickly to minimize poor clinical outcomes.
• Documentation: Maintain precise records of clinical, functional, and fiscal outcomes throughout the management process.

RN FIELD CASE MANAGER | REQUIREMENTS:

• Education: Associate’s or Bachelor’s Degree in Nursing (or a related field).
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Licensure: Must hold a current, unrestricted Registered Nurse (RN) license in Virginia.
• Certification: Case Management certification is required.
• Clinical Skills: Ability to analyze complex medical information and make rational, evidence-based decisions.
• Preferred Skills: Oasis assessments, Wound care and Wound Vac, IV therapy and lab draws, Chronic disease management
